Reimbursement Scheme Ontozry® (12.5mg/25mg, 50 mg, 100 mg, 150mg)
The health insurer does not fully reimburse ONTOZRY® (12.5mg/25mg, 50 mg, 100 mg, 150mg). The producer of this medicine has therefore asked TBR Nederland to reimburse the costs of the Personal Contribution to the patient.
Conditions
The Personal Contribution for ONTOZRY® (12.5mg/25mg, 50 mg, 100 mg, 150 mg) that is demonstrably charged to the patient by the health insurer as of the 1st of April 2022, will be fully reimbursed to a maximum of € 250,- per patient per year. ONTOZRY® 200mg is fully reimbursed by the health insurer and for this no co-payment is applicable.
Who can submit a claim
Only patients or their carer can claim the Personal Contribution for ONTOZRY® (12.5mg/25mg, 50 mg, 100 mg, 150 mg) online or by post. Online will be the fastest option. Pharmacists cannot claim this medicine on behalf of the patient.
